About the Opportunity
We’re seeking an experienced and compassionate Nurse Practitioner (NP) to join a respected Long-Term Care Home in Kitchener, Ontario.
In this pivotal role, you’ll provide comprehensive primary care, clinical leadership, and ongoing support to residents within a collaborative care model. You’ll work alongside physicians, nurses, and interdisciplinary team members to deliver high-quality, person-centered care that enhances quality of life and clinical outcomes.

Key Responsibilities

Provide comprehensive assessment, diagnosis, and management of residents’ acute and chronic health conditions.

Collaborate with the interdisciplinary team to develop individualized care plans aligned with residents’ goals and clinical best practices.

Support clinical staff through education, mentoring, and leadership in evidence-based care delivery.

Conduct medication reviews, manage prescriptions, and monitor for side effects or interactions.

Communicate effectively with residents, families, and caregivers to ensure transparency and quality of care.

Participate in quality improvement, infection control, and regulatory compliance initiatives within the LTC home.

Promote a culture of safety, compassion, and continuous improvement in resident care.
Qualifications

Registered Nurse (Extended Class) / Nurse Practitioner (NP) in good standing with the College of Nurses of Ontario (CNO).

Experience in long-term care, geriatrics, primary care, or chronic disease management strongly preferred.

Knowledge of Ontario Health LTC regulations, MOH documentation, and quality systems.

Strong leadership, communication, and organizational skills.

Ability to work independently and collaboratively in a multidisciplinary team.
