About the Opportunity

We’re seeking an experienced and compassionate Nurse Practitioner (NP) to join a respected Long-Term Care Home in Kitchener, Ontario.

In this pivotal role, you’ll provide comprehensive primary care, clinical leadership, and ongoing support to residents within a collaborative care model. You’ll work alongside physicians, nurses, and interdisciplinary team members to deliver high-quality, person-centered care that enhances quality of life and clinical outcomes.

Key Responsibilities

  • Provide comprehensive assessment, diagnosis, and management of residents’ acute and chronic health conditions.

  • Collaborate with the interdisciplinary team to develop individualized care plans aligned with residents’ goals and clinical best practices.

  • Support clinical staff through education, mentoring, and leadership in evidence-based care delivery.

  • Conduct medication reviews, manage prescriptions, and monitor for side effects or interactions.

  • Communicate effectively with residents, families, and caregivers to ensure transparency and quality of care.

  • Participate in quality improvement, infection control, and regulatory compliance initiatives within the LTC home.

  • Promote a culture of safety, compassion, and continuous improvement in resident care.

Qualifications

  • Registered Nurse (Extended Class) / Nurse Practitioner (NP) in good standing with the College of Nurses of Ontario (CNO).

  • Experience in long-term care, geriatrics, primary care, or chronic disease management strongly preferred.

  • Knowledge of Ontario Health LTC regulations, MOH documentation, and quality systems.

  • Strong leadership, communication, and organizational skills.

  • Ability to work independently and collaboratively in a multidisciplinary team.
